Try looking for the number on a payslip, benefit awards letter, pension book; tax statement; or tax return. If you still can't find it, contact your nearest Benefits Agency office or Inland Revenue office. If you can't find the number now, you can still send this form. But you will need to supply the number before you can move to a new home.
